A fitter attended a site to undertake some minor running repairs to an excavator mulcher. Having parked up unseen by the operator, he then entered the danger zone and was injured by flying debris.

This recent accident has highlighted the importance of ensuring that all visitors to site are aware of the site rules, risk zone/safety distances, PPE requirements and the arrangements for communications with operators on site before approaching any active operations.
